Fill out the online form with your contact information and reason for your appointment.

Wait for Our Call

Our office will contact you to schedule your Tele-Medicine appointment and send the log-in information you will need to speak with a doctor using your phone or computer. If you are using a desktop computer you will need a camera and microphone.

Step 3

Speak with the Doctor

Meet virtually with a doctor to discuss your symptoms and concerns.
