Payment Options

First Health Medical is a self-pay medical practice, which allows us to keep our pricing transparent and our care focused entirely on you. We do not accept medical insurance, but we happily accept the following forms of payment at the time of service:

Debit cards
Credit cards (a 3.5% processing fee applies to credit card payments)

New Patient forms

New Patient Forms are for new, first-time patients. If you have been to First Health Medical in the past 24 months your information will be on file. You do not need to switch primary care providers to be seen by a physician at First Health Medical.
